He is survived by his husband Jeremy Slade-Bloodworth of Clarksville; his mother Mary Bloodworth of Hendersonville, Tennessee; two sisters Beth Holmes of Gallatin, Tennessee and Terri Bloodworth of Hendersonville, Tennessee and a special friend Randy Talent of Kentucky.
