As we prepare for the 2023-2024 school year, Caldwell Medical Associates Tiger Clinic will be offering FREE annual sports physicals on May 24 & 25, 2023 from 4:30p.m.- 6:30p.m. Appointments must be scheduled in advance by calling 270-365-1225.
All surrounding counties are welcome!
Please obtain a KSHAA form from your school and bring it to your appointment completed.
