Jennie Stuart Health Free Christian County Sports Physicals

Jennie Stuart Health will provide Free Sports Physicals for student athletes, Saturday, June 19th, 8:00am – 10:00am in Lyle Dunbar Gym at Christian County High School. All Kentucky student athletes are required to have an up-to-date Kentucky High School Athletic Association Sports Physical annually.
In order to make the session more timely, each student athlete must complete the first four pages of the Sports Physical Information Form before arrival. Information to be completed includes:
Athlete Information
Student Athlete and Parent/Guardian Acknowledgement
Student Athlete Health History
The Sports Physical Information Forms to be completed, may be picked up from the front offices of Christian County High School and Hopkinsville High School daily.
