JSH/CCPS Vaccine & School Physical Clinic

Christian County Public Schools and Jennie Stuart Health have entered into a community partnership that will create three Vaccine and School Physical Clinic opportunities at JSH Family Health facilities. Interested families can call 270-707-2100 (Option 1) to make an appointment. If there is no answer, leave a message and someone will get back in touch with you.
These clinics will afford parents/guardians of CCPS students an opportunity to ensure the health of their child through well-child exams and vaccinations. The second clinic (with Dr. Jan Floyd) will be held from 8am-12pm Noon on Friday, Aug. 5 at 105 Keeton Drive in Hopkinsville, KY.
The Kentucky Department of Education requires physical exams, vaccinations, and other health-related documentation for all school-aged children. A list of requirements by grade/age can be found at: https://docs.google.com/…/1dNE6KQH0J83BepfGp9PwO9L…/edit
Jennie Stuart Health will also be on-site at Back-to-School Open Houses for those wishing ask questions and/or schedule an appointment in-person.
